For Your Benefit The Personnel Department would like to introduce this new column to discuss some of the benefits of employment at Douglas College and, in addition, to discuss the employee benefits resulting from employment at Douglas College. This month we will discuss filling in a HEALTH EXPENSE STATEMENT. This form is used when claiming for prescription drugs and other covered expenses, excluding eyeglasses. A HEALTH EXPENSE STATEMENT (white form) is available in the rack just inside the doors of the Personnel Office (Room 2300). This form has two sections: Part 1 is for general information and Part 2 is for the claiming information. Part 2 is designed to report the expenses of one claimant only. For each subsequent claimant, a new Part 2 must be filled out (see the reverse side of the form). Before you complete the form, take a photocopy of all your prescription receipts. COMPLETING THE FORM In filling out Part 2, take the information directly from the receipt(s). The prescription number is NOT the DIN NUMBER. Some of the new prescription receipts have a 1/4 inch peel-off strip, with all the information, that can be attached directly to the form. Attach a photocopy of all the prescription receipts to the form and submit it to Great West Life. GWL currently reimburses at 80% of the prescription amount in excess of the $25 annual deductible fee. Keep the original of the prescription receipts for submission to Pharmacare should you become eligible to claim. Pharmacare will only accept original receipts as proof of purchase. To claim for Pharmacare, you must have exceeded $600 per family per calendar year in eligible expenses. Once you have reached the $600 level, submit all of the original receipts for the calendar year along with a completed Pharmacare form available at the prescription counter of your pharmacy. To claim these expenses they must be submitted to Pharmacare by March 31 of the year immediately following the benefit year. Pharmacare currently reimburses at 70% of the prescription amount in excess of $600. Even after exceeding the $600 level, continue submitting the photocopies of the prescription receipts along with a HEALTH EXPENSE STATEMENT to GWL, because they will reimburse on the 30% that Pharmacare does not cover. The new provincial prescription system is now in place, and it may have impact on the procedure outlined above for coverage through Pharmacare.
