as TO: FACULTY & STUDENTS FROM: BRIAN PULHAM (AUDIO-VISUAL LIBRARIAN ) RE: FIIM & EQUIPMENT BOOKING PROCEDURES The following is a brief clarification ‘of procedure for booking films and equipment through the Douglas College libraries: 1) Film catalogues are available at all campuses to aid faculty & students in choosing films. The most complete set of catalogues is housed at New Westminster, the headquarters of A.V. Services. 2) When a film request is received and booked at New Westminster, form L7322 is filled in. A master copy is kept for records, one copy is sent to the instructor for confirmation, and one copy is sent to the campus library where the film is to be picked up. When a student or instructor picks up a film they must sign the campus copy of the form which carries a state- ment of responsibility. 3) It is the responsibility of the instructor to book equipment on which to show films or any other software. 4) Each campus library holds a basic stock of A.V. playback equipment. A complete list of what is available is kept with each A.V. assistant. Requests may be made verbally or in writing to the A.V. assistant on any campus. 5) Every month, form L7154 (rev. 26.7.73) is filled out for each piece of equipment, as requests are made the name of the borrower and location where the equip- ment is to be used are entered on the card. When the borrower picks up the equipment, form L7324 is filled out, the borrower signs the form and is given a copy. This form has a state- ment of responsibility for the equipment. 6) Users of equipment must show that they have been trained in the use of equipment; if training is needed, arrangements can be made for training to be undertaken by A.V. library assistants. 7) A.V. assistants will accept bookings for a complete semester in advance, but confirmation will not be made until one month prior to the use of the equipment. _and Hamers wrapped it up for Douglas with his } t oo...
