: 8 August 19/5 TO: All Directors & Supervisors FROM: Personnel Department RE: The Reporting of Accidents to the Worker's Compensation Board Personnel concerned are reminded of the requirement for all personnel injured at work to "report as soon as possible to your Super- visor to ensure your accident is recorded." College report forms are available from the Personnel Office. On receipt of this report the Personnel Department will then make out the W.C.B injury report to be forwarded to the Board.
